Hemorrhoidopexy with a circular stapler. Experience with 65 cases at the ABC Medical Center

ABSTRACT
Objective: To present the initial experience in handling of grade III and IV hemorrhoidal disease by means of the circular stapler (PPH).
Setting: Third level health care center.
Design: Prospective, longitudinal, descriptive study.
Statistical analysis: Percentages as summary measure for qualitative variables.
Material and methods: Sixty-five patients with grade III-IV hemorrhoids in a 2-year period. Patients were questioned at 12, 24, 48, and 72 h after surgery and at 7 days to assess pain in general and pain when defecating, using the Mankoski scale, presence of bleeding, return to daily activities, and satisfaction with the procedure.
Results: The main symptomatology was bleeding (75.38%) and prolapse (63.07%). At 24 h, eight patients (12.3%) required pain medication on schedule (Mankoski 4 or higher); at 48 h, 90% did not need pain medication (Mankoski 3 or lower). During the first bowels emptying, 17 (26.15%) patients referred pain of 4 or higher in the Mankoski scale; hence, they needed pain medication. The main observed complication was bleeding in 5 (7.69%) patients.
Conclusions: The results show that this is a safe procedure offering the patient an almost pain-free, faster recovery with prompt return to daily activities.
