Premature orofacial aging syndrome (POAS): diagnostic and treatment strategies

ABSTRACT
The complete loss of teeth, known as edentulism, causes significant transformation in the orofacial region of patients, inducing resorption and atrophy of the jaws. These changes not only affect masticatory function but also generate a series of functional and aesthetic alterations, such as dysphagia, dyslalia, respiratory disorders, apneas, and modifications in facial morphology. In response to this complex array of signs and symptoms, the concept of premature orofacial aging syndrome (POAS) is introduced. This syndrome encompasses various physical, functional, aesthetic, and psycho-emotional aspects associated with complete tooth loss combined with other pathologies. This article provides a comprehensive review of POAS, addressing its definition, etiology, and clinical manifestations. The importance of understanding the repercussions of POAS on quality of life is highlighted, addressing both aesthetic and psychosocial problems. Therapeutic approaches that combine maxillary advancement techniques and implant placement without bone grafts, along with aesthetic procedures such as rhytidectomy and rhinoplasty, are discussed. A retrospective study of six patients with an average follow-up of 56.5 months shows promising results, high implant survival, and patient satisfaction, significantly improving their self-esteem. The proposed treatments offer a viable alternative to improve the function of the stomatognathic system and the quality of life of patients with POAS, effectively addressing both aesthetic and functional needs. This article emphasizes the importance of health professionals recognizing this syndrome and the need to address it as a public health issue, promoting comprehensive and interdisciplinary care.
