Clinical-radiographic evaluation of surgical treatment in diaphyseal tibia fractures in pediatric population

ABSTRACT
Introduction: fractures of the tibial shaft are the third most common and the second cause of hospitalization. The mechanism is low and high energy. The male sex predominates, its treatment is controversial from conservative to surgical, with several alternatives existing.
Objective: to evaluate clinically and radiographically the surgical treatment, with the Karlström and Olerud criteria of surgical therapeutics used in pediatrics over 10 years of age, according to the implant used; from January 2018 to March 2023.
Material and methods: retrospective, cross-sectional study. 26 patients. Inclusion criteria: Pediatric patients aged 10-17, diagnosis of tibial fractures, operated with elastic intramedullary nailing (ESIN), plate, central medullary nail (CMN) and external fixator (EF). Exclusion criteria: open fractures, multiple fractures.
Results: 3 groups are obtained: ESIN (n = 10), plates (n = 12), CCM (n = 2) and 2 isolated cases, FE (n = 1) and compression screws (n = 1). The postoperative functional evaluation of patients with ESIN and plate showed a median of 32 and 30 points, respectively, (p = 0.238). The mean age in patients undergoing ESIN was lower than in patients with plate use, 12.6 and 14.5 years (p = 0.009).
Conclusions: satisfactory consolidation and functionality of moderate to good, a 12 weeks' time was identified for the start of walking, there is an overlap of one-year in age between the use of the ESIN and plate. Its indication determined by age and the 80 percentile, leaving the option of complementing the use of ESIN with other implants
