Antimicrobial susceptibility and resistance profile in pediatric patients with Gram-positive cocci infection in a referral hospital in the state of Aguascalientes, Mexico

ABSTRACT
Introduction: there is a great advance in the knowledge of the mechanisms of antimicrobial resistance, including genetic bases and determinants related to its dispersion. It is influenced by the use of the interpreted reading of the antibiogram as the basis of the step for its recognition.
Objective: to describe the susceptibility and resistance to antimicrobial agents and to identify the diagnoses and infectious syndromes against gram-positive cocci in pediatric patients at the Centenario Hospital Miguel Hidalgo, over a 5-year period.
Material and methods: this is a descriptive, observational and retrospective study, evaluated from January 1, 2018 to August 31, 2023, in patients aged 0 to 17 years, hospitalized, with confirmed infection by gram-positive cocci, from antibiograms report with susceptibility profile. Clinical and paraclinical data for evaluation of gram-positive infection were reviewed in each clinical record.
Results: 294 patients with positive cultures were reported. The main agents isolated were S. epidermidis with 43.20%, S. aureus 24.15%, E. faecalis 15.65% and E. faecium 6.80%. In Staphylococcus spp. and Streptococcus spp. infections, oxacillin resistance was reported at 54.42%. Annual resistance of S. aureus for oxacillin: 28.57% in 2018, 43.75% in 2023, sensitivity of 100% for doxycycline, in the period studied. Infectious diagnoses with the highest prevalence: bacteremia with 26.19%.
Conclusions: the marked increase in the incidence of bacterial resistance found in our study obliges us to implement regulations to modify its progression.
