Comparison of cytological diagnosis of fine needle aspiration biopsies and ultrasonography findings of thyroid nodules with the definitive post-thyroidectomy histopathological diagnosis

ABSTRACT
Objective: To compare the cytological diagnosis of fine needle aspiration biopsies and the ultrasonographic finding of thyroid nodules with the definitive post-thyroidectomy histopathological diagnosis in patients from the Hospital Español de Mexico.Materials and Methods: An observational, cross-sectional, descriptive, retrolective study, which reviewed patients’ records at the Hospital Español de Mexico with thyroid nodules who underwent fine-needle aspiration biopsy and/or ultrasonographic study prior to thyroidectomy in 2019 and 2020. Results of the aforementioned presurgical studies were compared to the definitive post-thyroidectomy histopathological result.
Results: Three study groups were formed: 23 patients who underwent fine needle aspiration biopsy and thyroidectomy, 37 patients who had an ultrasound study prior to performing thyroidectomy, but no fine needle aspiration biopsy was performed, and 25 patients who had fine needle aspiration biopsie and ultrasound prior to surgical procedure. All patients with a Bethesda classification I, II and III had a benign post-thyroidectomy report. Of the Bethesda IV classification, 28.5% were reported as malignant. All patients with TI-RADS 1, 2 and 3 had a benign post-thyroidectomy report. Of the TI-RADS 4 classification, 23% were reported as malignant, 100% of TI-RADS 5 had a post-thyroidectomy report of benignity, there were no reports of TI-RADS 6.
Conclusiones: The higher the Bethesda classification, the greater the risk of having a definitive malignancy result. No relationship was found between the results of the TI-RADS ultrasonographic classification with the definitive post-thyroidectomy histopathological result.
