Embryonic Reduction as an Option for Multiple Pregnancy of Triplets or Higher-Order of Pregnancies

ABSTRACT
Introduction: As of 2017, in our country the therapeutic option of embryonic reduction is offered to pregnant women with multiple pregnancy of triplets or higher order, referred from the Assisted Reproduction Center of Ramón González Coro Hospital, from the other three assisted reproduction centers in the country, or abroad. This procedure is carried out in the Provincial Department of Genetics of Havana.Objective: To characterize the fetal or multifetal reduction treatment of women with triplets or a higher order multiple pregnancy.
Methods: A descriptive, retrospective and longitudinal study was carried out including 14 pregnant women with pregnancies of triplets or higher order who underwent reduction of the number of fetuses in the Provincial Center of Medical Genetics in Havana, from January 2017 to December 2020.
Results: Pregnant women with advanced maternal age constituted about 50% of the sample. In vitro fertilization was the predominant assisted reproduction technique. The largest number of the sample corresponded to triplet pregnancies, in which gestation was extended to 35 weeks and more, but not in quadruplets where it was necessary to reduce with an interval of one week to reach the end of the pregnancy.
Conclusions: Despite the limited number of patients in the sample, the risk-benefit of this therapy should be assessed, as well as the alternative of offering it to all pregnant women with triplets or more who request it, with prior informed consent.
