ABSTRACT
Parkinson's disease is the most common, but not the only one, form of parkinsonian syndrome, which is characterized by bradykinesia, rigidity, and postural instability. The objective of this article is to highlight the importance of the differential diagnosis between the two diseases in order to adapt the subsequent rehabilitation treatment. We report the case of a patient initially diagnosed with Parkinson's disease. After a year of treatment with levodopa with no improvement and together with the atypical clinical data for this disease, the possible diagnosis of multisystemic atrophy is finally reconsidered. In conclusion, small nuances in the physical examination determine the treatment and make it possible to refine the final diagnosis. From Physical Medicine and Rehabilitation, the quality of life and global functionality of the patient suffering from multisystemic atrophy can be improved.
