Bilateral acetabular fracture secondary to seizure. Case report and literature review

ABSTRACT
Introduction: acetabular fractures are generally related to high energy mechanisms. A case report and literature review are exposed. There are few publications with reports of bilateral acetabular fractures secondary to seizures, a total of 26 case reports, no citations in the Mexican population.
Objective: to present a case of bilateral fracture of the acetabulum associated with convulsive crises, its management and clinical evolution. As well as a review of the literature and injury mechanims.
Clinical case: a 68-year-old female patient who debuted with an episode of seizures, presenting with a bilateral fracture of the AO acetabulum type 62B3.3/Judet, I. Performing surgical management, through open reduction and internal fixation using a modified Stoppa approach with two plates. 9-hole curved reconstruction plates for the left acetabulum and one open reduction and internal fixation with two 10-hole reconstruction plates and one 6-hole spring plate for the right acetabulum, presenting excellent functional results.
Conclusion: the injury mechanims of acetabular fractures continues to be of high energy, with few cases associated with seizures reported in the literature. Inevitably the patient will evolve to a post-traumatic coxarthrosis; the initial management with osteosynthesis and acetabular reconstruction is focused on the future definitive treatment with bilateral total hip arthroplasty with primary components, so far the result is encouraging.
