Deprescription in arterial hypertension, is it possible? A case report

ABSTRACT
Antihypertensive treatment is not without harm and sometimes the balance between the prevention of cardiovascular complications and treatment associated with adverse events is not easy. Although it is commonly advised to prolong therapy for life, different publications suggest that antihypertensive medications can be reduced or be phased out in some well-selected patients whose blood pressure is well controlled over an extended period. This process is called deprescribing and consists of withdrawal of one or more problematic or unnecessary medications, supervised by a health care professional with the goal of managing polypharmacy and improving the patient's quality of life and survival. We present the case of a patient with a 9-year history of hypertension who was being treated with a combination of antihypertensives and other drugs for diabetes and dyslipidemia and who, after three years of good BP control and subsequent weight reduction, was gradually deprescribed all his medications. In his last evaluation he maintained acceptable BP and adequate metabolic profile.
