Histopathologic diagnosis of a papillary renal cell carcinoma with lung metastasis

ABSTRACT
Introduction: Renal cell carcinomas accounts for 90% of all renal malignancies. Patients with this diagnosis present flank pain, hematuria and palpable abdominal mass. However, most diagnoses are made by incidental imaging findings.
Objective: To demonstrate the importance of immunohistochemistry for the diagnosis of renal carcinomas with cellular characteristics of difficult differentiation and their metastasis.
Case presentation: The case is reported of a 53-year-old man who presented paresthesia of the lower limbs. Radiological imaging revealed an intramedullary tumor and, incidentally, a pulmonary tumor and a renal cystic image. An initial lung biopsy was performed followed by a histopathological study of the renal cyst through histochemistry, which showed a poor differentiation of the analyzed tissue. Immunohistochemical markers were also determined: CK7, AMARC, RCC, CD10, EMA, CK20, TTF1 and CD117.
Results: The immunohistochemical evidence allowed defining the diagnosis of papillary renal cell carcinoma type 2 with lung metastasis.
Conclusions: This studied showed how the immunohistochemistry technique defines the type of renal cell carcinomas with little differentiation and their metastases.
