Association of allergic rhinitis, mites and obstructive sleep apnea-hypopnea syndrome in children. Cases and controls study

ABSTRACT
Introduction: the association of allergic rhinitis (AR), mites and obstructive sleep apnea-hypopnea syndrome (OSAHS) is a health problem that can occur at different ages. In AR and OSAHS, Dermatophagoides pteronyssinus (Dp), Dermatophagoides siboney (Ds) and Blomia tropicalis (Bt) are mainly involved.
Objective: to identify patients with rhinitis who are sensitized to mites and which have been diagnosed with OSAHS.
Material and methods: 120 individuals were selected from the allergy registry of the Previsora Polyclinic, Camagüey, Cuba, with a presumptive diagnosis of AR and ages between five and 15 years. All had performed the Prick test with Dp, Ds and Bt; some also, cardio-respiratory polygraphy (PCR) with the ApneaLink Air
TM device (ResMed Corp., RFA), using the device's automatic event marking. The sample consisted of 40 cases (those with oral breathing and snoring) and 80 controls (without oral breathing or snoring). The EPIDAT 3.1 program was used to study possible outcomes in cases and controls, using the χ
2 test for association and p < 0.05. Also simple diagnostic tests and basic statistical indicators. These values were estimated according to the apnea-hypopnea index (AHI) obtained by PCR.
Results: the mean size of the wheal was 6.7 mm in the cases and 4.8 mm in the controls (p = 0.31). The AHI was positive in 55 patients (45.8%); of them, 34 cases (28.3%) and 21 controls (17.5%). The sensitivity and specificity of the PCR showed values that allow confirming the degree of efficacy of the test to diagnose OSAHS. The patients with OSAHS in the cases and controls present an increase in number, with a predominance for cases (p = 0.002).
Conclusions: PCR with the ApneaLink device allows the diagnosis of OSAHS in RA.
