Chronic kidney disease in Mexico: challenges and proposals

ABSTRACT
Introduction. Chronic kidney disease (CKD) is one of the main causes of morbidity and mortality in Mexico. Its impact on affected individuals and their families is well understood. In order to reflect on the challenges posed by the current reality, a comprehensive analysis of the situation in Mexico was carried out, focusing on unmet needs and contrasting with internationally reported data to conclude with proposals based on evidence. Body. People with CKD face a convoluted, uninformed, and limited process, which is the result of a fragmented, insufficient, and costly healthcare system. In the authors´; opinion, the main challenges are timely access to preventive and substitutive treatment (including kidney transplantation), truly informed consent, the option of conservative treatment, education as the foundation for patient and family care, quality of life as a determining factor, and consideration of costs. The proposals mainly focus on changes in public health policies to meet the needs of care. Conclusion. CKD presents a public health problem in Mexico that requires immediate—and no less complex—action. The authors propose a radical change toward a model of care which is centered on the person and involves healthcare professionals as well as public decision-makers and policymakers. This model aims to be more cost- efficient and, most importantly, to improve outcomes in quality of life for people with CKD.REFERENCES
