Preschool follow-up using the cleft patient program: nursing process

ABSTRACT
Introduction. The Nursing Care Process (NCP) was applied to a girl with special needs, a user of secondary specialty care in the South of Chile, due to malformations of the oro-nasal cavity and genetic conditions with a medical diagnosis: Cleft of the hard palate and soft palate Robin syndrome and ankyloglossia, and surgical history of palatorrhaphy in 2019. Objective. To describe the application of the nursing process to preschoolers during their first check-up in the polyclinic at the end of 2022 of the Cleft Program recently implemented in the Hospital of origin). Method. Inés Astorquiza’s Human Needs Model as a disciplinary reference framework to apply the nursing care process. Assessment: nasal tone of voice, mouth breathing, sleep apnea, mandibular retrognathia and presence of caries are identified. The priority state of need “breathing”. Diagnosis: Ineffective respiratory pattern (00032) r/c craniofacial anatomical alterations m/p sleep apneas. Planning: NIC 7310-Nursing care upon admission and 5240-Advice: present the Fissurados program to the parents of the preschool. Execution: the Cleft Program is entered, hours are assigned and advice is given on controls with specialists for the management of the ineffective respiratory pattern: otolaryngology, audiometry, speech therapy, pediatric dentistry and dental care. Evaluation: process partially achieved, emphasis is placed on the nursing management role. Conclusion. The relevance of efficient nursing management is evident to provide continuous, timely services with high quality standards.
