Heart rate variability as an early predictor of mortality in patients with sepsis in the Intensive Care Unit

ABSTRACT
Introduction: the LF/HF index has been related to increased severity of illness and the mortality rate of patients with sepsis. The heart rate variability can identify early the risk of adverse outcomes and death in septic patients.
Material and methods: a case-control study with OR estimation for mortality and establishing a cutoff in the heart rate variability such as a mortality predictor.
Results: we determined the LF/HF index with a cutoff of 1.7, considering in the literature to predict mortality with OR 32.3 (95% CI = 1.1 to 914; p = 0.042). Through a ROC analysis, we identified two cutoffs in our patient's group, one in 1.08 and the other in 1.4, both with a sensitivity of 100% and specificity of around 70%. The risk analysis showed that values over 1.08 and 1.4 were risk reduction estimators with OR 0.43 (95% CI = 0.26 to 0.7; p < 0.001) and 0.45 (95% CI = 0.3 to 0.72; p < 0.001).
Conclusion: there is an association between the loss of heart rate variability and the mortality risk in patients with sepsis.
