Cholinergic urticaria: a rare and underdiagnosed pathology

ABSTRACT
Cholinergic urticaria is described as the presence of urticarial lesions on the skin, which appear in response to performing some activity that generates an increase in temperature in the human body, such as exercise, intense emotions or bathing with hot water. It represents 30% of all cases of inducible urticaria, and 5% of all cases of chronic spontaneous urticaria. One of the theories proposed for its development is that this condition is due to stimulation of the cholinergic nervous system. Although the exact pathophysiology is still uncertain; it has been observed that acetylcholine stimulates the degranulation of mast cells and the release of inflammatory mediators, so treatment must be aimed at modulating the inflammatory response. In this article we report the case of a 23-year-old woman who presented with sudden urticaria associated with physical activity. She reported eight years of evolution, having been treated with multiple medications, with a suspected allergic component, without a definitive diagnosis.
