Sensitivity and specificity of MRI with T1 sequence and contrast medium in the evaluation of muscularis propria involvement in rectal cancer

ABSTRACT
Introduction: magnetic resonance imaging (MRI) is recommended in the evaluation of patients with rectal cancer. Controversy exists regarding the usefulness of contrast-enhanced T1-weighted sequences to determine tumor penetration into the rectal wall and mesorectal fascia.
Objective: to determine the sensitivity and specificity of contrasted T1-weighted magnetic resonance imaging to assess involvement of the muscularis propria in rectal cancer.
Material and methods: it is a diagnostic test study in patients with histopathologically confirmed rectal cancer, with retrospective analysis of their rectal magnetic resonance imaging with contrasted T1 sequence, interpreted by an observer.
Results: 48 patients were included, 21 women (44%), 27 men (56%), age 33 to 85 years, mean 57.29 years. With polypoid tumor in 35 cases (72%), located in the middle third in 14 patients (29%). The contrasted T1-weighted magnetic resonance imaging sequence identified involvement of the muscularis propria in 12 of the 13 cases with histopathologically confirmed involvement of the muscle and obtained a negative report in 14 of 35 patients without histological involvement, sensitivity 92.3%, specificity 40%, positive predictive value 36.4 % and negative predictive value 93.3%.
Conclusion: magnetic resonance imaging with contrasted T1 sequence showed a high negative predictive value.
