Giant Baker´s cyst. Case report

ABSTRACT
Baker's cyst (BC) was first described by Adams in 1840 as a widening of the bursa that communicates with the knee's joint. The term provides from Dr. William Baker who concluded that it's appearance was caused by intraarticular pathology or by rheumatoid arthritis. Produced by an unidirectional valve mechanism from the joint to the bursa. The cyst appears in cases with severe effusion. We present a case of a 77 years old female with progressive edema from left pelvic extremity accompanied by reduced mobility and incapacity to walk. At the physical exam we found left leg with edema from the knee to both malleoli, pain over popliteal fossa and decreased knee's range of motion. Magnetic resonance of the left knee reported Baker's cyst of big dimensions which couldn't be accurately defined nor dismiss other pathologies. We asked for magnetic resonance extension to the leg simple and contrasted which showed images that corresponded with a Baker's cyst that extends to distal third of the leg with dimensions of 31 × 4.6 × 3.9 cm with a volume of 289 cm
3. After the application of gadolinium didn't show enhancement which suggested other pathology. The patient was a candidate to conservative treatment because of previously cardiovascular pathology.
