Idiopathic osteolysis syndrome in the pediatric population: Gorham-Stout disease

ABSTRACT
Introduction: Gorham-Stout disease is a rare pathology, characterized by lymphangiomatosis and hemangiomatosis. The primary lesion of the spine is uncommon, presenting pain, weakness and altered sensitivity, even compression or spinal cord section. The diagnosis is challenging due to the large number of differential diagnoses and the scant information regarding its etiology, pathophysiology, and therapeutic management.
Objective: to report the Gorham-Stout disease that causes compression of the thoracic spinal cord in a pediatric patient, diagnosis and management.
Clinical case: a 12-year-old female patient with back pain, decreased strength and sensitivity of the lower extremities. Imaging studies confirmed the presence of a lesion at T10. Surgical decompression, corpectomy with mesh replacement, and transpedicular instrumentation were performed. Gorham-Stout disease was reported in biopsy, with partial neurological recovery in the immediate postoperative period, without progressive improvement at six months.
Conclusions: Gorham-Stout disease should be considered in the differential diagnosis in localized neurological impairment. There is no established management protocol, treatment must be individualized in order to avoid complete or partial neurological damage, reversible or not.
