Psoriasiform ungual distrophy cause by chronic use of gel nails: a case report and review

ABSTRACT
Nails cosmetics have become an increasing cause of nail disorders
including traumatic onycholysis, onychodystrophy, contact
dermatitis, and infections. The chronic use of Shellac
TM can cause
psoriasiform nail dystrophy. Therefore, a thorough history might
help reach a timely diagnosis and give a rapid intervention. A
29-year-old female with a 20-day evolution of an asymptomatic
dermatosis with a history of chronic use of Shellac
TM, presented
all fingernails with subungual hyperkeratosis, longitudinal ridges,
oil spots, splinter hemorrhages, and onycholysis. The diagnosis of
psoriasiform onychodystrophy induced by chronic application of
Shellac
TM manicures was made, and the patient was advised to
suspend gel nail manicures, and treatment with topical clobetasol
0.4% ointment under occlusion once daily was prescribed for ten
days. After three cycles the patient referred complete resolution of
her dermatosis. This nail disorder may be the consequence of nail
polish components, instruments, or products used for its removal.
Swift suspicion of this condition might help clinicians give proper
education and treatment to prevent long-term nail damage.
