Reconstruction of the nasal columella with advancement flap of the upper lip

ABSTRACT
The reconstruction of the nasal columella represents a challenge for the plastic surgeon due to the complex anatomy of the nose as well as its function and aesthetic importance. The objective in this paper is to present a step-by-step procedure for the reconstruction of the columella with an advancement flap of the upper lip in a patient with facial trauma that caused loss of tissues of the right nasal ala and absence of vestibular skin, exposure of cartilage, lower wing and quadrangular cartilage and septum "in flag". The first surgical procedure was performed with the placement of a free graft in the nasal tip. However, the patient had late complications of nasal tip ptosis, external and internal nasal valve with retraction and collapse, retracted columella, septum with deviation to the right without obstruction, which is why he was scheduled for rhinoseptumplasty. We designed an upper lip advancement flap achieving reconstruction in a single surgical time with an adequate aesthetic and functional result.REFERENCES
