Reduction mammoplasty without a vertical scar

ABSTRACT
Reduction mammoplasty without a vertical scar is a procedure similar to inverted-T scar reductions, in which only the periareolar and submammary scars remain. It has two components: a deepidermized inferior pedicle flap, that will provide content and projection to the breast, and a superior skin flap, that will provide the skin cover. The latter is advanced on the inferior flap, adapted and sutured to a submammary incision. We present a retrospective review of this technique in 37 patients with breast hypertrophy. The reduction was bilateral in 30 cases and unilateral in seven cases. A total of 67 breasts were operated on, out of which, in 49 breasts, an inferior pedicle flap was used incorporating the nipple-areola complex, that is exteriorized through a circular hole made in an appropriate position of the superior skin flap; and, an inferior pedicle flap and a free nipple-areola graft were used in 18 breasts. The median follow-up period was 14 months. Postoperative complications were infrequent and mild, avoiding those that can develop at the junction of the vertical and horizontal incisions of the inverted T. The shape obtained was usually satisfactory. The horizontal scar is hidden under the breast, giving it an appearance of not having been operated on when the patient looks in front of a mirror. In conclusion, we consider that the procedure described is a good alternative to inverted-T scar techniques, achieving satisfactory results in severe breast hypertrophy.
