ABSTRACT
Introduction: recurrent pneumonia is a frequent problem in children with Down syndrome, due to greater susceptibility to respiratory tract infections (due to deficient immune system, hypotonia of the respiratory tract, anatomical alterations, hormonal alterations, etc.).
Clinical case: we present the case of a patient with Down syndrome, congenital hypothyroidism, congenital diaphragmatic hernia, which together condition the presence of recurrent cases of pneumonia.
Conclusion: in children with Down syndrome who present data of recurrent pneumonia and who, in addition, in the radiological study persistent opacities are observed, the possibility of a defect of the diaphragmatic wall should be considered. In the case presented, they allowed the diagnosis and treatment of recurrent pneumonia, however, the presence of a diaphragmatic defect was not taken into consideration, which together with hormonal alterations contributed to modify the motility of the airway and increase the accumulation of secretions, thus inducing changes in both pulmonary pressures and distensibility. As part of the follow-up of a patient with Down syndrome, it is imperative to ensure that adequate hormonal profiles are maintained, since the association of these entities is an indirect risk for good pulmonary function.
