Allergic Fungal Pansinusitis in an Immunocompetent Adolescent

ABSTRACT
Introduction. In recent years, reports of children without immunological compromise with Allergic Fungal Rhinosinusitis
(ARSF) are more frequent; its origin is related to the allergic response mediated by hypersensitivity to
various fungal agents in the nasal passages and paranasal sinuses without mucosa invasion.
Clinical case. 10-year-old male, consulted for decreased visual acuity without a favorable response, three months
later an increase in the volume of the internal canthus in the left orbit, epiphora and worsening of visual
acuity, secondary to rhinosinusitis. CT showed a multilobulated, heterogeneous mass in the sphenoid sinus and
in the right nostril in areas III and IV of Cottle, the left maxillary sinus being 95% occupied. Surgical extraction of
it was performed, the histopathological examination evidenced in PAS and Grocott staining: thin septate hyphae
suggestive of
Aspergillum spp. Specific treatment was given.
Conclusion. The clinical presentation in the pediatric stage differs from that in adults, in that the majority of
children present unilateral facial deformity with a history of benign chronic rhinosinus disease.
