Modeling infiltration disease influenced by bullying in a teenager patient

ABSTRACT
Cosmetic facial augmentation procedures using injections of different substances is common and has been increasing because it has come to be considered low risk, with good results and low cost; however, this is not always the case. Self-injection or injection of authorized or unauthorized materials or homemade formulas by personnel lacking anatomical knowledge and risks of the procedures has led to an increase in the incidence of multiple complications reported in the literature. Among the most common are deformities, granulomas, and infections, as well as the latent risk of skin necrosis. We present the case of a 15-year-old woman with a history of borderline personality, who reported having been psychologically abused at school because of the shape of her nose. She was admitted to hospital after having injected coconut oil and omega 3 with an insulin syringe in her nasal dorsum, nasal tip, lower lip and cheekbones. Given clinical data of regional inflammation, conservative management was started based on the literature and based on a multidisciplinary decision with corticosteroids and a double antibiotic scheme. Due to the satisfactory evolution of conservative and anti-inflammatory management, no surgical procedure was required. The symptoms were limited to 5 days, presenting gradual clinical improvement. We conclude that multidisciplinary management is essential in each patient, especially when there is psychological or psychiatric disorder.
