Thomasen osteotomy for sagittal imbalance due to ankylosing spondylitis

ABSTRACT
Introduction: Ankylosing spondylitis is a chronic inflammatory disease of unknown etiology and which mainly affects the axial and appendicular skeleton, with a tendency to fibrosis and progressive bony ankylosis, associated with postural limitation and disability.
Objective: To show that Thomasen osteotomy is an effective procedure for correcting and managing sagittal imbalance caused by ankylosing spondylitis.
Case presentation: Pedicle subtraction osteotomy (Thomasen) was performed on the middle lumbar spine (L3) with the objective of restoring sagittal balance as close to normal as possible. No deaths were reported.
Conclusions: Good functional outcome was obtained, with a high degree of patient satisfaction and improvement in their quality of life. This procedure is one of the options to be considered in spinal corrective surgery, with satisfactory outcomes.
