Pedicle subtraction osteotomy in the correction of thoracolumbar kyphosis due to ankylosing spondylitis

ABSTRACT
Introduction: Evidence shows the relationship between sagittal balance and quality of life in adult patients with thoracolumbar kyphosis. Thus, correction in the sagittal plane has become a primary objective in the surgical treatment of these ankylosing spondylitis conditions. Vertebral osteotomies allow to release the spine for this correction. However, they are complex techniques with a high rate of complications.
Objective: To assess the efficacy and safety of pedicle subtraction osteotomy in the correction of thoracolumbar kyphosis due to ankylosing spondylitis.
Methods: A descriptive, retrospective and longitudinal study was carried out. The sample consisted of 16 patients operated on, by using the aforementioned surgical technique, in the orthopedic service of Hermanos Ameijeiras Clinical-Surgical Hospital during the period from March 2001 to March 2021.
Results: All patients were male, had white skin and their average age was 39 years. There was a predominance of thoracic kyphosis with loss of lumbar lordosis. More than 80% of correction in sagittal plane was achieved, together with an average 30% of post-surgical lumbar lordosis. The most frequent complications were abdominal pain and surgical wound infection.
Conclusions: The use of pedicle subtraction osteotomy is an effective and safe technique for the correction of thoracolumbar kyphosis due to ankylosing spondylitis.
