Factors associated with the success of extubation in critically ill adult patients with COVID-19: cohort study

ABSTRACT
Introduction: invasive mechanical ventilation is a therapeutic tool that facilitates treatment in intensive care units (ICU), its greatest use in patients with acute respiratory failure (ARI). This study aims to describe the clinical variable related to the success of extubation in critically ill adult patients with COVID-19.
Material and methods: retrospective study, carried out at a reference clinic in Bogota from March 2020 to February 2021. The study was conducted in patients over 18 years of age with a diagnosis of SARS-CoV-2, COVID-19 with admission to ICU with a stay of at least 24 hours. Wilcoxon test and Fisher's test were performed.
Results: we included 125 patients with age median 60 years (IQR 50-65). We found that prolonged stay in the ICU (p < 0.001), sedorelaxation (p = 0.023), mechanical ventilation in spontaneous mode (p < 0.001), and hyperlactatemia (p = 0.046), are variables related with the failure of extubation.
Conclusion: the success of the extubation of the study population was 79% considering certain ventilatory tests that contributed to it; the failure of the extubations was associated with patients with prolonged stays use of neurorelaxant, and hyperlactatemia.
