Study of rhinophyma treated with high frequency electrosurgery: report of 20 cases

ABSTRACT
Background: rhinophyma is a dermatological condition considered
the last and most serious expression of rosacea. Phymas are
characterized by slow and progressive growth of the sebaceous
glands and connective tissue, causing severe anatomical alterations.
Surgical treatment options: partial or full-thickness excision
with or without skin grafting or flap reconstruction, dermabrasion,
cryosurgery, electrosurgery, and CO2 laser. High-frequency electrosurgery
is one of the methods in which satisfactory results
have been obtained.
Material and methods: we present a prospective observational
study of 20 patients with rhinophyma of various degrees of severity
treated with high-frequency electrosurgery, whose objective is
to evaluate the cosmetic results and the level of patient satisfaction.
Results: among the results we found 14 (70%) patients, with
excellent cosmetic remodeling and six (30%), with good results.
The level of satisfaction reported by the patients was very satisfactory
in 18 cases (90%), and two (10%) were satisfied. Complications
associated with the procedure were: hypopigmentation and
persistent erythema in four (20%), followed by telangiectasias in
three (15%) and hypertrophic scarring in two (10%); and none in
eight (40%).
Conclusions: radiofrequency electrosurgery is a safe, low-cost
method and, above all, with good aesthetic results, which is why
we consider it an excellent tool for treating phymas, particularly in
the nasal area.
