Dengue shock and polyserositis in infants

ABSTRACT
Introduction: The clinical manifestations of dengue virus infection have wide spectrum that varies from a mild viral infection to a severe life-threatening disease.Objective: To describe the clinical - hematological characteristics and the evolution of two infants with shock due to dengue and polyserositis.
Case report: Two infants who had shock and polyserositis as a result of dengue virus infection are reported here. The first case was a female eight months old patient, who was admitted for suspected dengue fever with a three-day history of fever and rash. After the first 48 hours, she had diarrhea and vomiting. She was diagnosed with the presence of warning signs and she was transferred to intensive care where she developed shock and polyserositis. She was treated according to the established protocol. After the critical stage of the disease, she progressively recovered and was discharged. The second case was a 5-month-old female infant hospitalized due to hyperthermia and acute seizure. She had fever with no other symptoms; on the sixth day, she showed signs of peripheral hypoperfusion and large abdominal distension, with polyserositis. She was transferred to the hospital for suspected dengue shock; she was treated according to the protocol. The signs of shock were reversed and she evolved satisfactorily.
Conclusions: The main clinical-hematological characteristics found were hemoconcentration, thrombocytopenia, hypoalbuminemia, and signs of organ dysfunction. In both infants, the evolution was favorable.
