Tomographic findings in suspected cases of COVID-19 and their association with the CO-RADS scale in patients with a negative RT-PCR test

ABSTRACT
Introduction: since COVID-19 pandemic in 2020, early diagnosis was crucial for the treatment. RT-PCR is the standard for the diagnosis of infection and may be limited in a certain group of patients, so chest tomography plays an important role in early detection.
Objective: determine the prevalence of chest tomographic findings in patients with clinical suspicion of COVID-19 and negative RT-PCR result and their association with the CO-RADS scale.
Material and methods: a cross-sectional, observational, retrospective study was carried out in patients with clinical suspicion of COVID-19 and negative RT-PCR test by nasopharyngeal swab, subsequently undergoing diagnostic chest computed tomography. According to the Dutch Radiological Society, the tomographic classification was evaluated using the CO-RADS scale.
Results: a total of 113 patients were analyzed, predominantly male, with 60%, and the average age was 58.4 years. The main tomographic findings were ground glass opacities with 77%, followed by a "crazy paving" pattern and consolidation to a lesser extent. Regarding the relationship with the CO-RADS scale, the tests had statistical significance with a p < 0.05, finding that most of the patients were classified as having high suspicion of the disease, CO-RADS 5.
Conclusion: the main tomographic findings in patients with clinical suspicion of COVID-19 illness and a negative RT-PCR test are like reported in the literature, which is associated with the fundamentals in the CO-RADS scale, where a high suspicion of the disease is observed, despite a negative confirmatory RT-PCR test.
