ABSTRACT
We present a clinical case of a young male in the second decade of life who presented septic shock due to enterobacteria with extended resistance isolated in rectal swab requiring early sepsis campaign guidelines. Had severe pulmonary injury and markers of fibrinolytic activity, pulmonary perfusion is performed by nuclear medicine documenting pulmonary thromboembolism secondary to antithrombin III deficiency and Leiden factor V. It evolves favorably with recovery of ventilatory mechanics tolerating release of mechanical ventilation without compromise of oxygenation or ventilation so it is decided to transfer to internal medicine and hematology.
