Presentación de dos casos de esófago en martillo neumático

ABSTRACT
Introduction: jackhammer esophagus (Jackhammer) is a hypercontractile motor disorder of the esophagus. According to the Chicago classification version 3.0, it is defined by the presence of waves with integrated distal contractility greater than 8,000 mmHg / cm / s in 20% or more of swallows.
Objective: to present two patients with the diagnosis of pneumatic hammer esophagus.
Method: retrospective and descriptive study of two patients with pneumatic hammer esophagus. High-resolution esophageal manometry was performed in a sitting position, with a solid Unisensor catheter, an MMS system with 36 sensors spaced one centimeter apart.
Results: Both male patients, aged over 50 years and with symptoms of gastroesophageal reflux disease. In one case, two swallows were recorded with integrated distal contractility greater than 8,000 mmHg/cm/s and in the other, five swallows. Mean distal latency, contractile front velocity, and integrated resting pressure were normal; although in one patient the integrated resting pressure was greater than 15 mmHg in four of the hypercontractile swallows. Intrabolus pressure was greater than 19 mmHg in both patients.
Conclusion: pneumatic hammer esophagus can be associated with reflux disease and its diagnosis must be confirmed by high-resolution esophageal manometry.
