DRESS syndrome: the great mimicker. Literature review

ABSTRACT
Drug reaction with eosinophilia and systemic symptoms syndrome
(DRESS) is a severe cutaneous drug reaction characterized
by a skin rash, lymphadenopathy, hematologic abnormalities,
and multi-organ involvement. Allopurinol, anticonvulsants,
sulfonamides, and antibiotics are the most frequently implicated
drugs. The onset of clinical manifestations is usually two to
three weeks from initial drug exposure. Diagnosis is based on
a combination of clinical and laboratory findings. Hematologic
abnormalities include eosinophilia, neutrophilia, monocytosis,
and atypical lymphocytes; lymphadenopathy has been identified.
The most affected organ is the liver. Treatment varies
depending on severity. Most patients recover completely with
discontinuation of the offending drug. Systemic corticosteroids
are the treatment of choice. Support measures should be associated
to control symptoms such as itching, inflammation
control with topical and/or systemic medications.
