Report of a case of laryngeal foreign body, importance of radiographic views

ABSTRACT
In pediatrics foreign body aspiration is common. Severe respiratory distress is presented in a healthy child. We describe the case of a four-month-old infant who, while being fed, abruptly started coughing, dyspnea and cyanosis. He required oxygen therapy and upper airway aspirations in the polyclinic. He recovered partially and was transferred to the José Luis Miranda Pediatric Hospital in Villa Clara. As respiratory symptoms persisted, he was hospitalized with a diagnosis of bronchial aspiration and medical treatment was decided. When the clinical picture persisted without rates, laryngotracheobronchitis was interpreted and treatment was started. Anteroposterior chest X-ray showed a decrease in the laryngeal lumen and a linear radiopaque image, coinciding with the cervical midline, 1cm long and 2mm in transverse diameter. The lateral view of the neck showed an increase of the prevertebral space and stop of the air column at C3-C4 that reappears at C5. The radiopaque image visualized in the frontal view was not defined. It was decided to perform axial tomography which confirmed foreign body in the glottis space. Direct laryngoscopy was performed where the foreign body (bone) was observed in the glottis space and it was extracted. The patient was discharged on the eighth day of the procedure with satisfactory clinical evolution. The use of several radiographic views is recommended in case of suspicion of a foreign body in the airway.REFERENCES
