Anti-reflux device as an uncommon cause of acute abdomen in an elderly patient at consultation of Internal Medicine

ABSTRACT
Background: The Angelchik prosthesis is an old anti-reflux device that consists of a C-shaped silicone ring placed around the gastroesophageal junction. Among the complications of its use are migration, displacement, erosion, and dysphagia. The most common of these complications is dysphagia (reported rates 70% to 96%), followed by migration and finally erosion. In patients with an acute abdomen, the initial diagnostic impression is incorrect in up to two-thirds of patients. This syndrome is a frequent symptom in the elderly and represents 10% of the causes of consultation in emergency medical services.Clinical case: A 86-year-old female patient that was admitted to the Internal Medicine service with data suggestive of acute abdomen, caused by an anti-reflux device placed 30 years before.
Conclusions: In elderly patients as in others, the clinical history is the basis in the diagnostic approach and thus cure of the condition; in this type of patients the old surgical practices should not be left aside, which although no longer found within the current guideline recommendations, for a long time they were the mainstay of treatment. In the Internal Medicine consultation, where a large part of the population is elderly, the main causes of dysphagia are not related to surgical history, there is usually a predisposition for systemic suspicion.
