Chronic non-puerperal uterine inversion in postmenopausal women

ABSTRACT
Introduction: Uterine inversion of gynecological or non-puerperal cause is a rare condition, usually associated with uterine tumors and It constitutes a diagnostic and therapeutic challenge.
Objective: To contribute to the timely diagnosis and treatment of this rare condition in postmenopause.
Case report: We report the case of a 70-year-old woman, multiparous, with a diagnosis of uterine fibroid made 20 years ago, she had refused the operation at that time. The patient was admitted complaining of a mass protruding through her vagina associated with bleeding and pain. A grade 3 uterine inversion was diagnosed by gynecological and ultrasound examinations; surgical intervention was proposed. Total hysterectomy was performed by laparotomy. Uterine inversion secondary to submucous myoma was evidenced. The histopathological study confirmed the diagnosis and ruled out malignancy. The patient was discharged five days after the intervention with no complications.
Conclusions: Non-puerperal uterine inversion is a rare condition that challenges gynecologists, both for its diagnosis and for surgical management. This justifies the need to include it among the differential diagnoses in patients who come with a mass that protrudes through their vagina and the preparation of novice professionals in the main surgical techniques for definitive resolution.
