Lidocaine-sparing technique in dermatological scalp surgery: Report of a successful case

ABSTRACT
Introduction: dermato-oncological surgery on the scalp involves the excision and mobilization of large skin areas, with the inherent use of large amounts of local anesthetics.Clinical case: We describe the case of a 55-year-old woman, diagnosed with a micronodular basal cell carcinoma, who underwent excision and reconstruction during the same surgical time using a novel technique of local anesthesia.
Discussion: using this anesthetic technic, we were able to mobilize 900 cm2 of scalp area with only 9 ml of lidocaine/epinephrine and 31.5 ml of saline solution, achieving an excellent anesthetic effect, an equivalent dose of 2.6 mg/kg of lidocaine.
Conclusion: this case provides evidence in favor of extensive dermatological surgery, which eliminates the risk associated with hospital admission, systemic anesthesia, or conscious sedation.
