Maternal Mortality at the Mexican Institute of Social Security, 1992-2001

ABSTRACT
From 1992 to 2001, Maternal Mortality Committees of the Mexican Institute of Social Security (IMSS) registered 2,578 maternal deaths. In this article, we report trends, causes, and factors associated with this maternal mortality, to conform an epidemiologic panorama of this health problem in our institution. During the period analyzed, the general maternal mortality rate decreased from 47.9 to 36.6 per 100,000 live births and specific mortality rate due to abortion decreased from 3.9 to 2.5; 67.7% of deaths were due to preeclampsia-eclampsia, obstetrical hemorrhages, abortion, and puerperal sepsis. Thirty one of 178 deaths (17.4%) caused by abortion were due to complications of illegal procedures (provoked abortion). Age-related mortality from abortion was higher in women 35 years of age and older (4.8 per 100,000 live births), whereas the lowest rate was found in teenagers (2.1); 23.6% of abortion deaths occurred in women during the first pregnancy and 46.6% in women during the third or subsequent pregnancy.
It is concluded that maternal mortality decreased among women attended at the IMSS, without important changes in type, causality and associated factors. Information suggests an insufficient level to date in medical care, which requires additional studies to better understand this problem and to identify feasible solutions.
