Awake patient intubation with achalasia diagnosis

ABSTRACT
Esophageal achalasia is a pathology of unknown etiology. In this, there is a loss of peristalsis at the level of the distal esophagus associated with a failure in the relaxation of the lower esophageal sphincter. The most common manifestation related to this pathology is solid dysphagia, followed by that associated with liquids and later by the regurgitation of soft foods or saliva. These patients are considered to have a full stomach and may present difficulties during anesthesia. To adequately perform orotracheal intubation and reduce patient discomfort, an awake intubation is proposed that includes: adequate sedation and local anesthetic, either by instillation in the pharynx and hypopharynx, or blockade of the superior and recurrent laryngeal nerves.
