Hashimoto encephalopathy: a rare case of encephalitis

ABSTRACT
The evaluation of a patient presenting with generalized neurological symptoms, such as encephalopathy, represents a challenge for the attending physician in terms of differential diagnosis, establishing the etiology and giving a proper management. We present the case of a 46-year-old man with a history of primary hypothyroidism who was admitted for a generalized tonic seizure. During hospitalization he developed neuropsychiatric symptoms. He was found to have elevated thyroid stimulating hormone, positive anti-thyroid peroxidase antibodies, alterations in the electroencephalogram, cerebrospinal fluid inflammation, and absence of brain structural injury by magnetic resonance imaging. Once infectious, autoimmune, and paraneoplastic causes had been ruled out, the diagnosis of Hashimoto's encephalopathy was concluded. He started treatment with methylprednisolone and intravenous immunoglobulin G with a good clinical response.REFERENCES
