Black esophagus: necrotizing esophagitis is an infrequent entity, and not well considered

ABSTRACT
The black esophagus is an entity described since 1967, but it was not until 1990 that its endoscopic characteristics were described. The publications of clinical cases and series of clinical cases defined the black esophagus as a pathological entity with clinical and prognostic implications, whose proper management improves the survival of the patient who suffers from it. Black esophagus is characterized during endoscopy by the presence of generalized and circumferential blackish pigmentation in the esophageal mucosa. Its etiology is unknown, multifactorial, involving a combination of factors, such as tissue hypoperfusion, gastroesophageal reflux, and decreased defense mechanisms, leading to acute necrosis. Clinically, it is manifested by upper gastrointestinal bleeding, dysphagia, epigastric pain, and chest pain. Diagnosis is discussed, which in the majority is made by an endoscopy study, in the same way the treatment of patients, where it is based on suppression of gastric acid, as well as correcting the predisposing medical condition. In conclusion, black esophagus is a rare entity, but it is a clinical syndrome whose importance lies in the differential diagnosis and its early recognition and control of predisposing factors improves morbidity and mortality.
