Clinical experience with microvascular venous couplers in free flaps

ABSTRACT
The aim of this paper is to report our experience with the use of the coupler device for venous anastomoses in patients undergoing reconstructive procedures with free flaps. We conducted a retrospective review of 14 microvascular flaps performed between April 2018 and May 2020. All venous anastomoses were carried out with the coupler device and arterial anastomoses were done manually. The data collected were age, sex, diagnosis, free flap type, anastomosis time and complications. Fourteen free flaps were realized: 5 anterolateral thigh (33%), 3 deep inferior epigastric artery perforator (20%), 3 peroneal (20%), 3 radial (20%) and 1 latissimus dorsi (7%). The average diameter of the couplers was 3 mm, with an average time to venous anastomosis of 3 min. No flap was lost due to venous failure. We conclude that the coupler device is a safe, easy-to-use tool with predictable results and should be considered a suitable alternative to traditional venous anastomoses.
