Expanding sublingual hematoma to deep neck spaces and airway compromise: emergency management

ABSTRACT
Background: Spontaneous hematomas in deep neck spaces are usually caused
by coagulopathies or chronic anticoagulant therapy. In some cases, they lead to lifethreatening
compromise of the airway that must be managed urgently.
Clinical case: A 49-year-old woman presented with gross enlargement of the submental
region and no signs of dyspnea on examination. The neck CT scan showed a
heterogeneous image with contrast enhancement in the sublingual, submental and
submandibular regions. Emergency airway management was ensured with a tracheostomy
and drainage of the deep neck hematoma.
Conclusions: Most hematomas in deep neck spaces develop as a result of large
vessel lesions or severe trauma. Surgical drainage of the hematoma must be performed
to avoid an airway compromise.
