Reliability of the Global OMERACT-EULAR Score System (GLOESS) in patients starting tumor necrosis factor inhibitors against active rheumatoid arthritis

ABSTRACT
Objective: To compare the clinical indices with the findings on ultrasound in patients with rheumatoid arthritis who started tumor necrosis factor inhibitors and assess their similarity.Materials and Methods: A prospective and single-blind study was done in Mexico City from March 2017 to August 2018. Adult patients aged 18 to 65 years with rheumatoid arthritis were included. Clinical indices and joints ultrasound were assessed based on the Global OMERACT-EULAR (GLOESS) score system recommendations, using gray-scale plus Power-Doppler. Patients were evaluated in three consecutive visits, with a six-week interval among visits.
Results: There were included 14 patients. At baseline 28-Joint Disease Activity Score- C-Reactive Protein was 3.275, then changed to 2.71 and 2.51 in the first and second visit, respectively. All other indices showed a decrease between baseline and first visit but not at second, while 7-joint count had no significative change. The ultrasound evaluation of 12 joints showed a significant correlation with the clinical indices.
Conclusions: Clinical indices have more ability to detect short-term differences and 12-joint ultrasound assessment with the GLOESS score system may be useful in the complex process of following in the treatment of patients with rheumatoid arthritis.
