Bilateral medial pale injury in generalized dystonia. Immediate and long-term response

ABSTRACT
Introduction. Changes in neuronal activity in the sensorimotor region of the Globus Pale Medial are considered fundamental alterations in the genesis of Dystonia. Injury to this region improves dystonia, but this effect has not been sufficiently studied.
Methods. Ten dystonic patients between 17 and 45 years of age underwent surgery between 2000 and 2002, injuring both Globus Pale Medial with neurophysiological guidance. The behavior of dystonia and the inability to activities of daily living was evaluated by the scales of Burke, Marsden and Fanh and of Activities of daily living (ADL of Schwan and England) respectively, before and 1, 3, 6, 9 12, 24 and 36 months after surgery.
Results. There is a gradual and significant improvement of the dystonic manifestations, with a reduction of the spasms that reached its maximum expression at 12 months. There were no intraoperative complications, nor long-term motor or neuropsychological affectations. Three patients worsened language.
Conclusions. Bilateral injury to the Globus Pale Medial in patients with dystonia was effective in reducing dystonia, with a gradual recovery profile that is essentially maintained until at least 3 years after surgery.
