Efficacy of parasargital translaminar epidural application of growth factors derived from Platelet Rich Plasma as a treatment for unilateral root pain caused by multisegmental disc disease

ABSTRACT
Introduction. In order to evaluate the efficacy of lumbar parasargital translaminar epidural administration of growth factors derived from Platelet Rich Plasma (FCPRP) to relieve unilateral root pain caused by multisegmental disc disease, a therapeutic clinical trial was conducted.
Methods. The patients were randomly distributed to two study groups: 5 ml of autologous ozonized FCPRP was applied to A, and 15 mg of bupivacaine and 40 mg of triancinolone were applied to B. Both procedures were performed on an outpatient basis, with local anesthesia, under fluoroscopic control, with a weekly frequency for two consecutive weeks. To evaluate the clinical improvement, the Lattinen test was used before, at one week, one month, three, six and twelve months. To evaluate the radiological improvement, the Pffirmann classification was used and the decrease of more than 50% of the size of the herniated fragment twelve months after the last treatment session. Adverse events occurred in both study groups were also identified.
Results: In both groups, male sex and age group under 45 years predominated (p> 0.001). The average value of the Latinen test was less than one
month in the FCPRP group and this trend was maintained until 12 months (p <0.001). There was no radiological improvement in any of the patients in the groups. The number of adverse events was five times higher in the group to which epidural steroids were applied. (p <0.001).
Conclusions: The application of epidural FCPRP was an effective interventionist method to relieve unilateral root pain caused by multisegmental disc disease.
