Incidental finding of microGIST during fundoplication: case report

ABSTRACT
Introduction: Some of the most common procedures in general surgery include the antireflux techniques but it is rare to detect extramucosal lesions in preoperative studies of these cases. One of the lesions that may exist in the general population, in up to 35%, is a gastrointestinal stromal tumor (GIST) of less than 2 cm and they are most commonly located in the stomach (60%). An observational conduct of these tumors is recommended if they are detected at the time of an endoscopy, unlike the recommended behavior in the event of a transoperative finding, where they must be resected, unless greater morbidity is added to the procedure, since even small GISTs with characteristics of low risk have the potential to recur and metastasize.
Clinical case: This is a 66-year-old female patient, who during antireflux surgery revealed a submucosal lesion in the gastric fundus, resection was performed, and the definitive histopathology reported a microGIST. There were no surgical complications and the patient had an uneventful postoperative course.
Conclusion: Small lesions should not be underestimated because they can present a malignant behavior, although infrequent. The general surgeon must be more aware of their existence, prognosis, and management in order to be able to approach this type of tumors appropriately if found incidentally.
