Chronic sinusitis and orbital complications associated with the use of cocaine: report of three cases

ABSTRACT
Cocaine is an alkaloid extract from the plant Erythoxylum coca mainly cultivated in South America. In 1912, Owen demonstrated the adverse effects of cocaine on the nasal tract, most commonly septal perforation. The intranasal abuse of cocaine causes local ische-mia, which translates to significant damage on bones and sinonasal soft tissue. This leads to a necrosis of the nasal septum, bone and cartilage necrosis, hard palate perforation and ulcers on the pharyngeal wall. The most common symptoms in cocaine users with nasal lesions are: nasal scabs, epistaxis, headache, hyposmia and mucopurulent rhinorrhea. Orbital and periocular complications derivated from the intranasal abuse of cocaine are uncommon but extremely important due to the repercussion on the quality of life of the patient. We present 3 cases with acute orbital involvement due to cocaine chronic sinusitis.
