Gigantomastia associated with pseudoangiomatous stromal hyperplasia

ABSTRACT
Gigantomastia is a pathology that we treat frequently, which is why it is important to know the different problems that we can face. We report a case of gigantomastia where the reduction mammoplasty access with the superior pedicle technique was modified, due to intraoperative findings, to a total mastectomy with free nipple grafting. The resection of 4.8 kg in the right breast and 3.4 kg in the left breast was performed. The final histopathological study with immunohistochemistry reported pseudoangiomatous stromal hyperplasia, a rare disease that can present in the breast in a localized, diffuse, unilateral, or bilateral manner. The patient was followed up for a year without performing reconstructive management, since pseudoangiomatous stromal hyperplasia has a high recurrence rate when complete resection of the breast tissue has not been performed. In this case breast reconstruction with fat grafting was performed in three surgical procedures.
