Point-of-care ultrasound accuracy for evaluating different diseases in the emergency service

ABSTRACT
Introduction: Point of care ultrasound is currently a fast and precise method to diagnose multiple diseases in the emergency department.
Objective: To evaluate the diagnostic precision of point of care ultrasound in comparison with the reference pattern of the reports of Radiology / Cardiology service.
Methods: A prospective study was conducted, from January 2017 to April 2018, with 459 patients who came to the Emergency Department, at Royal Bournemouth Hospital and Christchurch Hospital for various diseases. Sensitivity, specificity, as well as positive and negative predictive values were calculated. The interpretation of the emergency physician's ultrasound was compared with the gold standard of the reports from the Radiology (ultrasound) and Cardiology (echocardiography) services. The concordance kappa index was calculated for the analysis of the results.
Results: The sensitivity, specificity, positive and negative predictive values, and the precision index (kappa) of the point of care ultrasound were 92.3%, 100%, 100%, 96.3%, respectively; 0,94 for deep vein thrombosis; 94.4%, 98.6%, 94.4%, 98.6%, 0.93 for abdominal aortic aneurysm; 91.4%, 92.3%, 88.9%, 94.1%, 0.92 for left ventricular function; 93.8%, 91.73%, 88.2%, 95.7%, 0.89 for pneumonia; 93.3%, 92.3%, 87.5%, 96.0%, 0.84 interstitial syndrome.
Conclusions: The results obtained prove the precision of point of care ultrasound in the comprehensive evaluation of various diseases in the emergency department. Therefore, early interventions can be performed on patients.
